Each week, I’ll share my perspective on the evolving state of remote care programs in pharmacy and how we can move toward more continuous, connected care.
Traditional care is built around episodes, but chronic disease doesn’t behave that way. Most disease progression happens between visits: quietly, gradually, and often invisibly. Office encounters capture snapshots, but they rarely reveal patterns. That visibility gap is what Remote Patient Monitoring (RPM) is designed to solve.
